In an interview, the author of Jujutsu Kaisen, Gege Akutami revealed more details about the Prison Realm, for those who follow the manga know what this “Prison Realm” involves and who is there.
Anyway, the author answered some questions that fans have about the Prison Realm, and he revealed bizarre things, among the most bizarre things he revealed we have the following:
Question: It is said that there is no passage of time in the Prison Realm, but the prisoner doesn’t feel hungry? How do they use the bathroom?
Gege Akutami: Since physical time does not flow, eating and defecation do not happen. I didn’t really get a chance to talk about it in the manga, but jujutsu sorcerers have special “bugs” that they put in their stomachs for long-term missions when eating is difficult and bathroom breaks are tough.
Who is trapped in the Prison Realm is Satoru Gojo, who is the strongest character in the entire Jujutsu Kaisen manga, according to the author, inside the Prison Realm, Gojo is doing his best not to think about anything, because if he started thinking too much, he would eventually go insane.

And that because of that he needed to put Gojo in the corner so that the story could move and have conflicts, but I couldn’t find where I read that so don’t consider it 100% true.
